Q: What information is needed to file for disability benefits?
A: To begin, you need your Social Security number and the Social Security numbers of any of your dependants who may receive benefits. Certified copies of the birth certificates of all of these will be requested. All of the names, addresses and phone numbers where you received care, including all doctors, hospitals, clinics and other institutions, are mandatory, including the dates of all medical visits and tests. All of the medical records pertaining to your disability also need to be provided. The names and dosage of all medications you take need to be listed. A summary of your complete work history, including dates and descriptions of your duties for up to the past 15 years or more, are also needed as well as your most recent W-2 form.
